It is required that every Cyclone Swim School
participant have a signed release form in our possession
prior to any enrollee’s participation.
A parent AND a physician signature is required.
Please complete
the Release and Medical Authorization form for each
participant and bring it to your first day of class.
This release will cover all classes from May 1, 2010-April 30, 2011.
NO SWIMMER WILL BE ALLOWED TO PARTICIPATE WITHOUT THIS
FORM
The release and the treatment authorization must
be signed by a parent or guardian if student is under 18
years old. Students who are 18 years old or will become
18 years old before the end of the session must also
sign. In order for students to participate in Cyclone
Swim School lesson activities, this form must be signed
and on file prior to participation.
